dba BASES - Baseball And Softball Essential Skills, including its staff and agents.
Parents, please read & acknowledge this agreement on the registration form
I understand that if my child is injured during any BASES activity or event (camps, clinics, strength and conditioning prgorams, batting cage use) or related activity and should require medical attention, appropriate medical care will be summoned and/or ambulance will be called. I understand that physical activities engaged in during such events do carry some degree of personal risk and the potential for serious injury is always possible. I understand that it is very important to provide a thorough set of contact information including a reliable Emergency Phone and Emergency Contact Person that will be able to be reached during the hours of the event. If I and my Emergency Contact are not available, I understand that every effort will be made to contact me and to avoid delays in treatment. I consent to costs related to treatment (beyond those covered by my personal insurance). I give my authorization for my child to participate in the BASES activity or event, and hereby release, indemnify, and hold harmless ENFUCO Enterprises, LLC, BASES, its staff, and agents from any claim or liability for accident or injury that occurs while participating. I indicate my acknowledgement and understanding of this personal injury disclaimer and my sole financial responsibility for medical expenses that may occur by pressing the applicable Acknowledgement Button on the electronic registratrion form.

REFUND POLICY FOR OUTDOOR CAMP
Weather can affect our ability to hold camp on any scheduled day during outdoor only camps. When holding camp at a facility run by an independent organization (i.e. Anne Arundel County Parks & Rec, et al) camp will be cancelled when that organization closes the fields for any scheduled day. In the event that camp must be cancelled due to weather, the following refund policy will be applied:
NUMBER OF CANCELLED DAYS - 1 day NO REFUND; 2 days $50 refund; 3 days $75 refund; 4 days $100 refund; 5 days $125 refund
